They are in standard Microsoft Word format for you to download, print out, and complete. If your computer uses Microsoft Windows 98 or later, WordPad will read the files if you don't have the Microsoft Word program.

The forms referenced below can be completed prior to your first meeting with your attorney, not only to save time but also for you to keep in case you need the information later.

Will Preparation Questionnaire - This is a form for you to fill out to provide your attorney with the information to prepare a will.

Probate Information Form - This is a form to complete in preparation for the administration of a decedent's estate.
